個人情報保護方針
前澤化成工業株式会社（以下「当社」という。）は、個人情報取扱事業者として個人情報ならびに個人番号および特定個人情報（以下「個人情報等」という。）を適正に取り扱います。
当社は、個人情報等保護の重要性を認識し、「個人情報保護方針（プライバシーポリシー）」を制定して、「個人情報の保護に関する法律（以下「個人情報保護法」という。）」「行政手続における特定の個人を識別するための番号の利用等に関する法律（以下「マイナンバー法」という。）」その他関連法令および個人情報等に関するガイドライン（以下「ガイドライン」という。）に従い、個人情報等の適正な取扱いがなされるよう取り組んでいます。
- 法令・規程の遵守
当社は、個人情報等について、個人情報保護法、マイナンバー法その他関連法令およびガイドラインを遵守し、個人情報保護規程その他関連規程を整備のうえ、当社が保有する個人情報等の保護に努めます。
- 利用
当社は、個人情報の利用目的を当社の正当な事業範囲内で明確に定め、その目的達成のために必要な限度において、適正な方法で個人情報の取得、利用および提供を行います。

- 管理
当社は、保有する個人情報を「個人情報の利用目的」の達成に必要な範囲内で、正確かつ最新の内容に保つよう適切な管理に努めます。また、保有する個人情報等保護のため、実行性ある安全管理措置を講じます。
- 第三者への提供
当社は、法令上の要請による場合を除き、本人の同意を得ないで個人情報を第三者に提供しません。また、当社が個人情報を「個人情報の利用目的」のため外部へ委託する場合には、委託先に対し当社と同様に適切な管理を行うよう契約によって義務付けます。

- 情報漏洩の防止
当社は、保有する個人情報等の取扱いに当たっては、不正なアクセス、漏洩、滅失または毀損の防止その他安全管理のために必要かつ適切な措置を講じるとともに、問題が発生したときは、適切かつ速やかに対応します。
- 個人番号および特定個人情報に関する事項
当社は、個人番号および特定個人情報については、マイナンバー法で明記された目的以外のために取得および利用しないとともに、マイナンバー法で認められる場合を除き、個人番号および特定個人情報を第三者に提供しません。また、当社が個人番号および特定個人情報を外部へ委託する場合には、委託先に対し当社と同様に適切な管理を行うよう契約によって義務付けます。
- 問い合わせ
当社は、保有する個人情報等について、本人から開示、訂正、追加、削除、利用停止、消去もしくは第三者提供の停止の求め、または苦情もしくは問い合わせがあったときは、個人情報問い合わせ窓口にて適切に対応します。
- 個人情報保護方針の見直し
当社は、個人情報の取扱いをより適正なものにするため、この個人情報保護方針を適宜見直し、継続的な改善に取り組みます。また、改善した内容を、この方針に随時反映します。
個人情報の利用目的
当社は、個人情報を以下に掲げる目的以外には利用いたしません。
- お取引先様の情報の利用について
- 製品の見積書、請求書または注文書を送付させていただくため
- 製品を納入または受領させていただくため
- 製品の受発注等に関する進捗状況の報告または確認をさせていただくため
- お問い合わせ等に回答または利用させていただくため
- 各種資料を送付または受領させていただくため
- 製品やサービスに関する情報を提供または請求させていただくため
- 渉外に関する連絡、相談や配布物の授受または配布をさせていただくため
- お支払方法やご入金状況に関する確認のご連絡をさせていただくため
- 製品やサービスに反映させるためのアンケートを実施させていただくため
- 株主様の情報の利用について
- 会社法に基づく権利の行使や義務の履行をさせていただくため
- 株主様に対して、優待品の贈呈、事業報告書の送付、その他各種便宜を供与させていただくため
- 株主様を対象とするアンケートの実施等、ＩＲ活動に利用させていただくため
- 各種法令に基づき、所定の基準による株主様のデータを作成する等、株主様の情報を管理させていただくため
- 採用希望者（内定者を含む。）の情報の利用について
- 採用に関する情報提供および連絡をさせていただくため
- 採用選考の資料とさせていただくため
- 内定者情報として管理させていただくため
- 正式入社後の配属等を検討させていただくため
個人番号および特定個人情報の利用目的
当社は、個人番号および特定個人情報をマイナンバー法で明記された目的以外には利用いたしません。
個人情報の開示等請求手続きのご案内
当社に提供いただきました個人情報について開示等の請求を希望の方は、以下の手続きを確認いただきますようお願いいたします。

請求可能な事項および請求時に提出いただくもの
- 当社所定の請求書について
開示等の請求は、当社所定の書式（個人情報開示等請求書.pdf）をご利用ください。本書式以外による請求には回答できませんので、予めご了承ください。
- 本人確認書類について
ご本人様からの請求であることを確認させていただくために、本人であることを確認できる以下の書類（以下「本人確認書類」といいます）をご提出ください。
- 運転免許証のコピー
- パスポートのコピー（住所の記載があるものに限ります）
- 在留カードのコピー
- 障がい者手帳のコピー
- その他公的機関の発行する写真つき証明書（コピー可）
- 手数料
「個人情報の利用目的の通知」「個人情報の内容の開示」をご請求の方は、手数料をお支払いただくことが必要になります。手数料は郵便切手に限定させていただいておりますので、請求書に404円分の切手を同封していただきますようお願いいたします。（現金や定額小為替では受付できません）
- 提出先および問い合わせ先
書類のご提出は「特定記録郵便」など配達記録が残る方法を用いて、以下の個人情報問い合わせ窓口までご郵送ください。
なお、来社による請求はお受けできませんので予めご了承ください。
- 当社からの回答について
ご提出いただいた書類等に不備・不足がない場合は、確認書類に記載された住所宛に書面を郵送することで回答いたします。
なお、以下に該当する場合は、開示等の対応をすることができませんので予めご了承ください。開示等の対応をすることができない場合であっても理由を記載した書面を郵送いたしますが、手数料はお返しいたしません。
- 本人または第三者の生命、身体、財産その他の権利利益を害するおそれがあるため
- 当社の業務の適正な実施に著しい支障を及ぼすおそれがあるため
- 請求の手続きに不備があるため
- 開示することが法令に違反することとなるため
- 法令に特別の手続きが定められているため
- 訂正等の請求に理由があると認められないため
- 訂正等をすることにより当該個人情報の利用目的の範囲を超えてしまうため
- 利用停止等を行うことに多額の費用を要する場合その他の利用停止等を行うことが困難であるため
- 請求に関する個人情報の利用目的について
請求によりご提供いただきました個人情報は、請求の回答のためにのみ利用し、他の目的では利用いたしません。

Personal Information Protection Policy (Privacy Policy)
MAEZAWA KASEI INDUSTRIES CO., LTD. (hereinafter, the “Company”) properly handles personal information as well as Individual Numbers and Specific Personal Information (hereinafter, “personal information, etc.”) as a business operator handling personal information.
The Company recognizes the importance of protecting personal information, etc. and has established a “Personal Information Protection Policy (Privacy Policy)” and implements measures for the proper handling of personal information, etc. in accordance with the “Act on the Protection of Personal Information (hereinafter, “APPI”),” the “Act on the Use of Numbers to Identify a Specific Individual in Administrative Procedures (hereinafter, “My Number Act”)” and other relevant laws and regulations as well as guidelines concerning personal information, etc. (hereinafter, “Guidelines”).
- Compliance with laws and regulations and other rules
The Company shall endeavor to protect personal information, etc. held by the Company by complying with the APPI, My Number Act and other relevant laws and regulations as well as Guidelines concerning personal information, etc. and by maintaining personal information protection rules and other relevant rules.
- Use
The Company shall clearly prescribe the purpose of utilization of personal information within the legitimate scope of the Company’s operations and properly acquire, use and provide personal information to the extent necessary to achieve such purpose.
*Matters concerning Individual Numbers and Specific Personal Information are detailed in 6 below.
- Management
The Company shall endeavor to appropriately manage and maintain personal information that it holds so that the information is accurate and up to date within the scope necessary for the achievement of the “Purpose of utilization of the personal information.”In addition, effective safety control measures shall be implemented to protect the personal information, etc. that is held.
- Provision to third parties
The Company shall not provide personal information to a third party without obtaining the person’s consent, unless in the case of a request under laws and regulations.In addition, in the case that the Company entrusts personal information for the “Purpose of utilization of the personal information” externally, the outsourcer shall be contractually obligated to appropriately manage in the same way as the Company.

- Preventing information leakage
The Company shall take necessary and proper measures for the prevention of unauthorized access, leakage, loss or damage, and for other security controls when handling the personal information, etc. that it holds and respond appropriately and promptly when there is a problem.
- Matters concerning Individual Numbers and Specific Personal Information
The Company shall not acquire or use Individual Numbers and Specific Personal Information for purposes other than specified in the My Number Act and shall not provide Individual Numbers and Specific Personal Information to third parties other than cases authorized by the My Number Act.In addition, in the case that the Company entrusts Individual Numbers and Specific Personal Information externally, the outsourcer shall be contractually obligated to appropriately manage in the same way as the Company.
- Inquiries
The Company shall appropriately respond at the personal information inquiry desk concerning personal information, etc. held when a person requests disclosure, correction, addition, deletion, discontinuation of its utilization, erasure or discontinuation of its provision to a third party or when there is a complaint or inquiry.
- Amending the Personal Information Protection Policy
The Company shall appropriately review and continuously improve this Personal Information Protection Policy to ensure more appropriate handling of personal information.In addition, improved content shall be reflected in this policy at any time.
Purpose of utilization of the personal information
The Company shall not use personal information for any reason other than the following purposes.
- The use of customer information
- To send quotations, invoices and order forms for goods
- To supply or receive goods
- To report on or confirm the status of orders received, placed, etc. for goods
- To respond or use for inquiries, etc.
- To send or receive various documents
- To provide or request information concerning goods and services
- To contact for public relations purposes, to give and receive and to distribute materials and consultations
- To contact for confirmation of payment method and status of payment
- To conduct questionnaires for reflections on goods and services
- Use of shareholder information
- To exercise rights and to fulfill obligations in accordance with the Companies Act
- To present gifts, send business reports, and provide various other types of benefits to shareholders
- To use for IR campaigns such as conducting shareholder questionnaires
- To manage shareholder information, such as the creation of shareholder data according to prescribed standards based on various laws and regulations
- Use of the information of employment candidates (including provisional hires)
- To contact and provide information concerning recruitment
- To use as recruitment selection materials
- To manage as provisional hire information
- To consider for purposes such as assignment after formally joining the Company
Purpose of use of Individual Numbers and Specific Personal Information
The Company shall not use Individual Numbers and Specific Personal Information for purposes other than specified in the My Number Act.
Details on procedures for requests such as disclosure of personal information
We ask that persons wanting to make a request such as for disclosure of personal information provided to the Company, confirm the following procedures.

- About the Company’s prescribed request form
Please use the Company’s prescribed form（Request form for personal information disclosure.pdf）o make a request for disclosure.Please understand in advance that responses cannot be made to requests not made on this form.
- About identification documents
Please submit the following documents (hereinafter, “identification documents”) so that identification can be verified to confirm that the request is from the person concerned.
- Copy of your driver’s license
- Copy of your passport (only where your address is listed)
- Copy of your residence card
- Copy of your physical disability certificate
- Other certificates issued by public bodies that include a photo (can be a copy)
- Fee
Persons requesting the “Notice of the purpose of utilization of the personal information” and “Disclosure of personal information details” are required to pay a fee.Since the fee payment is restricted to postage stamps, please enclose stamps to the value of 404 yen in the envelope with the request form. (We are unable to accept cash or money orders.)
- Destination for submissions and contact point
Please use a method that enables registration of delivery such as “registered mail” and post the documents for submission to the personal information inquiry desk listed below.
Please note in advance that requests cannot be accepted by visiting the company.
- About the response from the Company
If there are no deficiencies or omissions in the submitted documents, the response will be made in writing by post to the address listed in the identification documents.
Please note in advance that we are unable to respond to disclosures, etc. in the following cases.Although a written notice detailing the reason will be sent by post even in the case that we are unable to respond to disclosures, etc., we will not refund the fee.
- Due to the risk of harm to the life, body, property or other rights or interests of the person or any third parties
- Due to the risk of serious impediment to the proper execution of the Company’s business
- Due to deficiency in the request procedures
- Due to disclosure violating laws and regulations
- Due to special procedures prescribed in laws and regulations
- Due to the reasons for requests such as correction not being recognized
- Due to correction, etc. changing such purpose of utilization of the personal information beyond the scope
- Due to the discontinuation of its utilization, etc. costing large amounts or otherwise difficult to discontinue its utilization, etc.
- The purpose of use of personal information concerning requests
Personal information provided with a request is only used to respond to the request and is not used for any other purpose.
Handling of personal information, etc. on the website

Although a Cookie is information saved by the browser when accessing the website, it does not include personal information such as name and email address.
Cookie information may be used for access analysis and to provide effective advertising to persons who have accessed this website.
